Infectious Disease Reporting

A list of over 80 contagious illnesses must be reported to the local Health Department by medical professionals, labs, and other designated sources.

Health care providers can reach an Infectious Disease Specialist during regular business hours (Monday through Friday from 8 a.m. to 5 p.m.) at 406-258-3896
After business hours, health care providers and emergency responders can reach an on-call Infectious Disease Specialist by calling 9-1-1. This contact option is for health care providers and emergency responders only.

Individual Infectious Disease Case Reporting

Healthcare providers must report all suspected and confirmed infectious disease cases on DPHHS’s Communicable Disease Reporting list. Fill out the appropriate form (see list below) and submit via the secure file transfer. If you need password assistance please call 406.258.3896.

Infectious Disease Outbreak Reporting

Several types of facilities are required to report when an outbreak occurs at their facility. This includes healthcare settings such as long term care facilities, hospitals, outpatient clinics, and surgery centers. It also includes community settings such as assisted living facilities, group homes, shelters, detention centers, schools, daycares, religious organizations, and community events.

There are two options for reporting. You only need to complete one.
