Nurse-Family Partnership

We know becoming a mom for the first-time can be a lot to handle, and the Nurse-Family Partnership program is designed to give you all the support you need.  At the heart of our program are caring nurses, who will personally help guide you through all the ins and outs of pregnancy, infancy, and parenting a toddler.   Your nurse will partner with you and share real-time support and practical advice every step of the way.

You can join if you are pregnant with your first child, and meet income requirements. Visits begin early in your pregnancy (no later than the 28th week of pregnancy) and completes when your child reaches 2 years old.

We would like to tell you more and answer all your questions. Contact us so we can get you connected to your own personal nurse.
